eDSD System

The Dynamic Support Register and optional Case Management tool supporting three types of referrals (professional, parent/carer and self-referral) into the DSR, and enabling multi-agency case management for enhanced support provision

Summary

The eDSD system, developed in partnership with Kent County Council, is a modern, user-friendly and streamlined Dynamic Support Register with optional Case Management tools. This system offers the opportunity to enhance the use of the register itself, as well as the support and case management of each individual listed on the register, with the ultimate aim to prevent unnecessary hospital admissions and detention under the Mental Health Act for people with a learning disability and autistic people.

The eDSD System enables three types of referral to enhance the professional referral process, but also to facilitate self-referrals and referrals by parents/carers via a publicly accessible form. In-depth case summaries for each individual on the DSR supports LDA teams and keyworkers in effectively monitoring the current circumstances of people with a learning disability and autistic people, enabling the provision of appropriate and timely support. Lastly, the eDSD System has been designed with multi-agency collaboration and communication in mind, offering a secure and centralised location for information to be shared and stored across multiple services and organisations.

Easy to navigate system

RAG Risk Identification

Red, Amber, Green and Blue risk identification in referral form, with ability to audit step up/down history in case summary

Holding area

Once automatically notified of referrals, cases appear in triage to assess risk, assign team members and add to DSR

Filterable DSR

Advanced filtering of the Dynamic Support Register to identify cases, e.g. by referral type, RAG risk, gender etc.
